Boca Home Care's priority is to ensure a person's ability to maximize their independence while at the same time securing a safe environment. As a Medicare certified home health agency, we provide, skilled services to patients needing further nursing care or therapy at home after discharge from a hospital, nursing home or rehab facility. Medicare home health can also be ordered by your doctor in lieu of an inpatient stay, or in case of an emergency. Boca Home Care arranges for medical equiptmet (wheelchair), oxygen, medications, bandages or other services needed at home as per your doctor's order. Upon receiving your physician's referral, our professional caring staff will schedule a home visit within 24 hours. An experienced, licensed nurse or therapist will visit to assess and begin treating the patient according to the order and care plan signed by your physician. We treat, teach and train the patient and caregiver to enhance recovery and prevent future problems. YOUR MEDICARE PART A BENEFIT COVERS RNs, THERAPISTS, AIDES AND MEDICAL SOCIAL WORK SERVICES IN YOUR HOME AT 100%. At Boca Home Care, we understand patients prefer remaining in their "orbit of care" therefore we work with all area physicians, hospitals, skilled nursing and rehab centers. Our staff and exclusive contractors participate in continuing education and training. The executive team meets regularly with doctors in our community to coordinate and exchange information to improve our quality of care.
